Search the Apple App Store or Google Play Store for CARE CONVENE PATIENT.<br \/>\n2. Select \u201cRegister Now\u201d and then \u201cI Will Register Myself\u201d<br \/>\n3. Complete the registration using the provider code FMSGR, this does have to be in all capital letters.<br \/>\n4. Look for an email from Care Convene. Click \u201cVerify my email address.\u201d It could be sitting in Junk Mail folder as well if it is not in your inbox.<br \/>\n5. Open the Care Convene app and enter your email and password to log in.<br \/>\n6. Complete your personal information before you need to use the app for care.<br \/>\n7. To add dependents: While in the app, tap the gear icon (Settings) on the far right. At the top of the screen, tap \u201cDependents.\u201d On the top right, tap \u201cadd or +\u201d and fill out the form. Once the form is completed, tap the \u201csave\u201d button.<br \/>\n8. Please call our office to schedule a telehealth visit with one of the providers.<br \/>\n9. At the time of you visit, or a few minutes prior, login and select \u201cVisit\u201d in the bottom menu and select Family Medicine practice.
