Telehealth sign-up:
1. Search the Apple App Store or Google Play Store for CARE CONVENE PATIENT.
2. Select “Register Now” and then “I Will Register Myself”
3. Complete the registration using the provider code FMSGR, this does have to be in all capital letters.
4. Look for an email from Care Convene. Click “Verify my email address.” It could be sitting in Junk Mail folder as well if it is not in your inbox.
5. Open the Care Convene app and enter your email and password to log in.
6. Complete your personal information before you need to use the app for care.
7. To add dependents: While in the app, tap the gear icon (Settings) on the far right. At the top of the screen, tap “Dependents.” On the top right, tap “add or +” and fill out the form. Once the form is completed, tap the “save” button.
8. Please call our office to schedule a telehealth visit with one of the providers.
9. At the time of you visit, or a few minutes prior, login and select “Visit” in the bottom menu and select Family Medicine practice. Please then choose the provider you are scheduled with to start your visit.
